What Is That Cracking Sound? Chiropractic Cavitation Explained

The single most common question from first-time patients, usually asked with a slightly nervous laugh: "What was that?" It is a fair question, and the answers people have picked up — bones grinding, a vertebra going back into place, cartilage wearing down — are all wrong. Here is what actually happens.

The Sound Is Gas

Your joints are enclosed in a capsule containing synovial fluid, which lubricates and nourishes the joint surfaces. Dissolved in that fluid are gases, mostly carbon dioxide and nitrogen — the same way carbon dioxide is dissolved in a sealed bottle of sparkling water.

An adjustment separates the joint surfaces very slightly and very quickly. That sudden increase in volume drops the pressure inside the capsule, and dissolved gas comes rapidly out of solution to form a bubble. That phase change is the sound. It is called cavitation.

Real-time MRI studies published in the last decade settled a long-running argument about this: the imaging showed the sound occurring at the moment a cavity forms, not when a bubble collapses. Either way, the sound is a gas event in fluid. Nothing is grinding, nothing is breaking, and no bone is moving back into a socket.

Nothing Goes "Back Into Place"

This deserves stating plainly because the language around chiropractic often implies otherwise. Vertebrae are not out of place in the sense of being dislocated — a genuine dislocation is a serious injury requiring emergency care, and you would know. What we are addressing is a joint that has lost its normal range of motion: it is restricted, not displaced.

An adjustment restores motion to that restricted joint. The improvement comes from the restored movement and the neurological effect it has on the surrounding muscles — not from anything being relocated.

Why the Sound Does Not Mean the Treatment Worked

This is the genuinely useful part. Patients often judge a visit by whether they got a good crack, and that instinct is misleading in both directions.

Plenty of highly effective treatment produces no sound at all. Mobilisation, instrument-assisted adjusting, drop-table technique, and soft tissue work are frequently silent and frequently what a particular patient needs — especially older patients, post-surgical patients, and anyone where low-force technique is indicated.

Conversely, a joint can cavitate loudly without the restriction actually being addressed, particularly if the segment that made the noise was not the segment that needed help. This is exactly why self-manipulation — twisting your own neck until it pops — tends to give a few seconds of relief and no lasting change. You are cavitating whichever joint is most mobile, not the one that is stuck.

Does Cracking Your Knuckles Cause Arthritis?

No. The most cited work on this is charmingly direct: a physician cracked the knuckles of one hand and not the other for over sixty years, then compared them. No difference in arthritis. Larger observational studies since have found the same. Habitual knuckle cracking has been associated with some grip strength differences in a few studies, but not with arthritis.

The One Thing Worth Avoiding

Repeatedly self-manipulating your own neck. Not because of the sound, but because of what it means: you are reaching for temporary relief from a problem that has not been diagnosed, and you are doing it by moving the joints that already move too much while the restricted one stays restricted. Over time that can increase instability in the hypermobile segments. If your neck constantly feels like it needs to crack, that feeling is a symptom worth investigating rather than satisfying.

If the Sound Bothers You
