What to Expect at Your First Chiropractic Visit

If you have never been to a chiropractor before, it is completely natural to feel uncertain about what the experience involves. Will it hurt? What will the doctor actually do? Do you need a referral? How should you prepare? These are questions we hear every week at Bromberg Chiropractic, and they deserve clear, honest answers.

Having welcomed thousands of first-time patients to our Cambridge practice over the past four decades, I can tell you this: the anticipation is almost always worse than the reality. Most new patients leave their first visit saying some version of "That was nothing like what I expected. I wish I had come sooner." Here is exactly what happens, step by step.

Before You Arrive

No Referral Needed

One of the most common misconceptions about chiropractic care is that you need a referral from your primary care physician. In Massachusetts, you do not. You can schedule directly with a chiropractor, and most insurance plans cover chiropractic services without requiring a referral or prior authorization. If you have questions about your specific coverage, our front desk team can help verify your benefits before your appointment.

What to Wear

Wear comfortable, loose-fitting clothing that allows you to move freely. Think athleisure, gym clothes, or anything with some stretch. You will be asked to perform certain movements during the examination (bending, twisting, raising your arms), and restrictive clothing makes that difficult. Avoid dresses, skirts, or stiff jeans if possible. You will not need to change into a gown for a standard visit.

What to Bring

Bring your insurance card, a photo ID, and any relevant medical records you have on hand. If you have recent imaging, such as X-rays, MRIs, or CT scans, bring those or have them sent to our office in advance. If you were referred by another provider, bring their notes. If you have none of these, do not worry. We can work with what you have.

Step 1: Health History and Intake

Your visit begins with paperwork, much like any medical office. You will complete a health history form covering your current symptoms, past injuries, surgeries, medications, family health history, and lifestyle factors such as your occupation and activity level.

Be specific and thorough. Details that might seem unimportant to you can be highly relevant to your chiropractor. For example, a fall you had five years ago, a car accident you walked away from, or the fact that you sit at a desk for ten hours a day. These details help us connect the dots between your history and your current symptoms.

After the paperwork, I will sit down with you for a one-on-one conversation about your chief complaint. I want to understand not just where it hurts but how the pain behaves: When did it start? What makes it better or worse? Is it constant or intermittent? Does it radiate? How is it affecting your daily life? This conversation typically takes 10 to 15 minutes and is one of the most important parts of the visit.

Step 2: The Physical Examination

Next comes the hands-on examination. Depending on your symptoms, this may include:

  • Range of motion testing: I will ask you to bend, twist, and move in specific directions while I observe your movement quality and note any restrictions or pain.
  • Orthopedic tests: These are specialized maneuvers designed to stress specific structures (joints, discs, ligaments, muscles) to help pinpoint the source of your pain.
  • Neurological testing: Reflex checks, sensation testing, and muscle strength assessments tell us whether any nerves are being compromised.
  • Posture and gait analysis: How you stand and walk reveals a great deal about structural imbalances, compensations, and areas of dysfunction.
  • Palpation: I will use my hands to feel the muscles and joints of your spine and the surrounding area, identifying areas of tenderness, spasm, swelling, or restricted movement.

The examination is thorough but not painful. You may experience mild discomfort when a sore area is palpated or a specific test reproduces your symptoms, but we will never push you beyond what you can tolerate.

Step 3: Diagnosis and Treatment Plan

Once the examination is complete, I will explain my findings in plain language. You will understand what is causing your pain, why it is happening, and what we can do about it. If I suspect a condition that requires diagnostic imaging, such as a possible disc injury or fracture, I will arrange for X-rays or an MRI referral before proceeding with treatment.

Your treatment plan will outline:

  • Your specific diagnosis
  • The recommended course of treatment
  • Approximately how many visits you are likely to need
  • What you can do at home to support your recovery
  • Any activities you should modify or avoid temporarily

I believe in transparency. You will know what we are doing, why we are doing it, and what results to expect. If I believe your condition would be better served by another type of provider, whether an orthopedist, neurologist, or physical therapist, I will tell you that directly and facilitate the referral.

Step 4: Your First Adjustment

In most cases, we can begin treatment on the same day as your initial visit. The chiropractic adjustment is the cornerstone of what we do. It involves applying a controlled, specific force to a joint that is not moving properly. The goal is to restore normal motion, reduce nerve irritation, and allow the body to function and heal as it should.

Here is what most first-time patients want to know about the adjustment:

Does it hurt? The vast majority of patients experience little to no pain during an adjustment. You may feel pressure and hear a popping or cracking sound. This is simply gas being released from the joint fluid, similar to cracking your knuckles. It is completely normal and harmless. Many patients report immediate relief after their first adjustment.

Is it safe? Chiropractic adjustments have an excellent safety record. Serious complications are extraordinarily rare. As a practitioner with over 40 years of experience, patient safety is always my first priority. I will never perform a technique that is not appropriate for your specific condition.

How will I feel afterward? Most patients feel better immediately: lighter, more mobile, and less tense. Some patients experience mild soreness in the treated area for 24 to 48 hours, similar to the feeling after a good workout. This is a normal response as your body adjusts to the corrected alignment. Drinking plenty of water and applying ice if needed can minimize any post-treatment soreness.

Step 5: Home Care Instructions and Follow-Up

Before you leave, I will give you specific recommendations for what to do between visits. This may include ice or heat therapy, stretches, postural modifications, ergonomic adjustments for your workstation, or activity restrictions. What you do between appointments matters as much as what we do during them.

We will also schedule your next visit. For acute conditions, this is typically within a few days. As you improve, visits become less frequent. Our goal is always to resolve your problem in as few visits as possible and equip you with the knowledge to prevent recurrence.

How Long Does the First Visit Take?

Plan for approximately 45 minutes to one hour for your initial visit. This allows adequate time for the history, examination, discussion of findings, and treatment. Follow-up visits are shorter, typically 15 to 30 minutes.
